The following issues have been fixed in the ITSM Patch-5.0.4.0608 - English release:
#40668 Be able to specify the search result form and grid
#4399 Tab Order incorrect on Incident form for Self Service
#40394 Dropdown values reverting to default when saving even though a different value is chosen within the web portal
#42099 BPEngine does not find Incidents created from an import that meet the past due requirements.
#40947 Category field blanks out when used in SelfService.
